How to Apply

​

1. Duly completed Application Form and executed License Agreement

2. Photography of NRIC-applicant (Principal & Supplementary)

3. Photocopy of birth certificate of NRIC for children above 18, if any

4. Photographs-2 copies photo each of applicant, spouse and children(Passport Driving license size)

5. Payment for Entrance Fee, Security Deposit and Processing Fee payable to KELAB GOLF DESA DUNGUN
